Registered Nurse - care coordination (Virginia Beach, VA)

Nova Mill Transit Planning Group helps transport agencies around Virginia Beach design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on service alerts with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with service alerts. • BLS/ACLS certification. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
