Registered Nurse - care coordination (Tulsa, OK)

Foxglove Basin Health Group provides primary and specialty care in Tulsa, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of about 828 people based in Tulsa. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on patient scheduling with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with patient scheduling. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Tulsa, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
