Registered Nurse - care coordination (Toledo, OH)

Fernwood Orchard Hospitality is a hospitality company in Toledo with restaurants, rooms and catering, and we need a Registered Nurse to lead our care coordination. Promotion from within is common. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on reviews and feedback with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with reviews and feedback. Working arrangement • Full-time, Monday to Friday. • On site in Toledo. • We reply to every applicant.
