Registered Nurse - care coordination (Toledo, OH)

Clearpath Stone Transit Planning is looking for a Registered Nurse. The job centres on care coordination for ridership counts, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Support work on ridership counts with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Toledo. • We reply to every applicant.
