Registered Nurse - care coordination (Toledo, OH)

We are growing our team at Clearpath Fox Pharma and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 61 people based in Toledo. What you will do • Support work on quality documentation with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with quality documentation. • BLS/ACLS certification. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Toledo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
