Registered Nurse - care coordination (Savannah, GA)

Rivermark West Care Services Group is looking for a Registered Nurse. The job centres on care coordination for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on care plans with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Savannah, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
