Registered Nurse - care coordination (San Jose, CA)

We are growing our team at Bluepeak Haven Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of more than 1,000 people based in San Jose. What you will do • Support work on complaint handling with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with complaint handling. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
