Registered Nurse - care coordination (San Jose, CA)

Westbrook Linden Health is looking for a Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in San Jose. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on patient education with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with patient education.
