Registered Nurse - care coordination (San Jose, CA)

Westbrook Early Learning is an early-learning provider in San Jose, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 86 people based in San Jose. What you will do • Educate patients and families in plain language. • Support work on licensing records with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with licensing records. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in San Jose. • Applications are reviewed on a rolling basis.
