Registered Nurse - care coordination (San Jose, CA)

Redstone Umber Transit Planning plans bus and rail service for cities around San Jose. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on ridership counts with clinical expertise. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Full-time, on a regular day shift. • On site in San Jose. • Applications are reviewed on a rolling basis.
