Registered Nurse - care coordination (San Jose, CA)

Amberfield Cross Transit Planning plans bus and rail service for cities around San Jose, and we need a Registered Nurse to lead our care coordination. Work is collaborative and map-heavy. What you will do • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on fare systems with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
