Registered Nurse - care coordination (San Jose, CA)

Lumen Harbor Transit Planning helps transport agencies around San Jose design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on accessibility audits with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
