Registered Nurse - care coordination (San Jose, CA)

Bluepeak Rowan Care Services is a social-care provider serving older adults and people with disabilities around San Jose, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on visit scheduling with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in San Jose. • We reply to every applicant.
