Registered Nurse - care coordination (San Diego, CA)

Northwind Clear Care Services provides in-home and community support in San Diego, and we need a Registered Nurse to lead our care coordination. The work is personal and so is the culture. What you will do • Support work on licensing with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with licensing.
