Registered Nurse - care coordination (San Antonio, TX)

Northwind Wren Care Services is a social-care provider serving older adults and people with disabilities around San Antonio, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 46 people based in San Antonio. What you will do • Support work on licensing with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Full-time, on a regular day shift. • On site in San Antonio, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
