Registered Nurse - care coordination (Salt Lake City, UT)

Clearpath Orchard Care Services Group provides in-home and community support in Salt Lake City. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on family communication with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • On site in Salt Lake City. • We reply to every applicant.
