Registered Nurse - care coordination (Salt Lake City, UT)

Ironwood West Care Services Partners is looking for a Registered Nurse. The job centres on care coordination for visit scheduling, and it would suit someone who likes steady, practical work. You would join a large organisation of about 243 people based in Salt Lake City. What you will do • Educate patients and families in plain language. • Support work on visit scheduling with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with visit scheduling. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Salt Lake City. • Applications are reviewed on a rolling basis.
