Registered Nurse - care coordination (Sacramento, CA)

We are growing our team at Amberfield Summit Learning Group and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on learning materials with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with learning materials. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Sacramento. • We reply to every applicant.
