Registered Nurse - care coordination (Sacramento, CA)

We are growing our team at Nova Mill Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 138 people based in Sacramento. What you will do • Support work on service alerts with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with service alerts. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Sacramento. • Applications are reviewed on a rolling basis.
