Registered Nurse - care coordination (Rochester, NY)

We are growing our team at Cedar River Early Learning and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of more than 1,000 people based in Rochester. What you will do • Support work on tuition and subsidies with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with tuition and subsidies. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Rochester. • We will tell you what to expect at each step of the hiring process.
