Registered Nurse - care coordination (remote)

Foxglove Lantern Medical Group is a Oakland company producing monitoring and surgical equipment. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of about 349 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on clinical evaluations with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with clinical evaluations. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Oakland. • Applications are reviewed on a rolling basis.
