Registered Nurse - care coordination (remote)

Clearpath Iron Medical is a Hartford company producing monitoring and surgical equipment, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 168 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Support work on device records with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with device records. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with an occasional day on site in Hartford. • We will tell you what to expect at each step of the hiring process.
