Registered Nurse - care coordination (remote)

Lumen Summit Learning Group Associates is an education provider in Fresno serving students and families. We are hiring a Registered Nurse to take on care coordination. Families are partners. What you will do • Follow infection-control and documentation standards. • Support work on attendance records with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with attendance records. • BLS/ACLS certification. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Fresno. • We reply to every applicant.
