Registered Nurse - care coordination (remote)

Rivermark Glen Health Group provides primary and specialty care in San Jose, and we need a Registered Nurse to lead our care coordination. Clinicians and administrators work side by side. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on referrals with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with referrals. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in San Jose. • We will tell you what to expect at each step of the hiring process.
