Registered Nurse - care coordination (remote)

Bright Mill Care Services is a social-care provider serving older adults and people with disabilities around Pittsburgh. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Support work on visit scheduling with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Pittsburgh. • Applications are reviewed on a rolling basis.
