Registered Nurse - care coordination (remote)

Rivermark Orchard Care Services is a social-care provider serving older adults and people with disabilities around Charleston. We are hiring a Registered Nurse to take on care coordination. Teams support each other, especially on hard days. What you will do • Educate patients and families in plain language. • Support work on licensing with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with quarterly visits in Charleston. • Applications are reviewed on a rolling basis.
