Registered Nurse - care coordination (remote)

We are growing our team at Clearpath Hollow Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. Work is collaborative and map-heavy. What you will do • Support work on ridership counts with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Denver. • Applications are reviewed on a rolling basis.
