Registered Nurse - care coordination (remote)

Nova Mill Transit Planning helps transport agencies around Richmond design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on route schedules with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with route schedules. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with an occasional day on site in Richmond. • We reply to every applicant.
