Registered Nurse - care coordination (remote)

We are growing our team at Ironwood Sparrow Health and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 130 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on referrals with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with referrals. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in Knoxville. • We reply to every applicant.
