Registered Nurse - care coordination (remote)

Bluepeak Orchard Health is a healthcare provider in Cincinnati with clinics and support teams, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of about 700 people, working mostly from home. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on patient scheduling with clinical expertise. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with patient scheduling. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with a monthly team day in Cincinnati. • We reply to every applicant.
