Registered Nurse - care coordination (remote)

Bluepeak Bridge Care Services is looking for a Registered Nurse. The job centres on care coordination for visit scheduling, and it would suit someone who likes steady, practical work. You would join a large organisation of about 759 people, working mostly from home. What you will do • Support work on visit scheduling with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Tucson. • We reply to every applicant.
