Registered Nurse - care coordination (remote)

We are growing our team at Cedar Meadow Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on training materials with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with training materials. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Hartford. • Applications are reviewed on a rolling basis.
