Registered Nurse - care coordination (remote)

Redstone Clear Transit Planning helps transport agencies around San Jose design routes and schedules. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on ridership counts with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with ridership counts. • BLS/ACLS certification. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in San Jose. • We reply to every applicant.
