Registered Nurse - care coordination (remote)

Lumen Harbor Transit Planning Group is looking for a Registered Nurse. The job centres on care coordination for ridership counts, and it would suit someone who likes steady, practical work. We like people who ride the bus and notice when things do not work. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on ridership counts with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with ridership counts. • BLS/ACLS certification. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Fort Worth. • We will tell you what to expect at each step of the hiring process.
