Registered Nurse - care coordination (remote)

Bright Harbor Transit Planning plans bus and rail service for cities around San Antonio, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 61 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Remote-friendly, with quarterly visits in San Antonio. • We will tell you what to expect at each step of the hiring process.
