Registered Nurse - care coordination (remote)

Clearpath Sparrow Care Services is looking for a Registered Nurse. The job centres on care coordination for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Milwaukee. • We reply to every applicant.
