Registered Nurse - care coordination (remote)

We are growing our team at Foxglove Forge Transit Planning Partners and adding a Registered Nurse whose main responsibility will be care coordination. We like people who ride the bus and notice when things do not work. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on fare systems with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, afternoons. • Remote-friendly, with a monthly team day in Charleston. • Applications are reviewed on a rolling basis.
