Registered Nurse - care coordination (remote)

Harborlight Hollow Care Services Group provides in-home and community support in Houston. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 135 people, working mostly from home. What you will do • Support work on visit scheduling with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Houston. • Applications are reviewed on a rolling basis.
