Registered Nurse - care coordination (remote)

Lumen West Care Services supports families and individuals in Madison with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 28 people, working mostly from home. What you will do • Support work on visit scheduling with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Madison. • Applications are reviewed on a rolling basis.
