Registered Nurse - care coordination (remote)

Highbridge Meridian Health Group is a healthcare provider in Richmond with clinics and support teams, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on patient scheduling with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with patient scheduling. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Richmond. • Applications are reviewed on a rolling basis.
