Registered Nurse - care coordination (remote)

We are growing our team at Ironwood Wilder Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Support work on device records with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with device records. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Reno. • We reply to every applicant.
