Registered Nurse - care coordination (remote)

Westbrook Birch Care Services provides in-home and community support in Columbus, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Support work on licensing with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Columbus. • Applications are reviewed on a rolling basis.
