Registered Nurse - care coordination (remote)

We are growing our team at Stonegate Sparrow Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on route schedules with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Tampa. • Applications are reviewed on a rolling basis.
