Registered Nurse - care coordination (remote)

Fernwood Juniper Transit Planning is looking for a Registered Nurse. The job centres on care coordination for accessibility audits, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Support work on accessibility audits with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Jacksonville. • We reply to every applicant.
