Registered Nurse - care coordination (remote)

Rivermark Lakeside Care Services provides in-home and community support in Savannah. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on visit scheduling with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Savannah. • We reply to every applicant.
