Registered Nurse - care coordination (remote)

Bluepeak Ashton Health provides primary and specialty care in Cincinnati, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on referrals with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with referrals. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Cincinnati. • Applications are reviewed on a rolling basis.
