Registered Nurse - care coordination (remote)

Greenridge Drift Health is a healthcare provider in Nashville with clinics and support teams. We are hiring a Registered Nurse to take on care coordination. Clinicians and administrators work side by side. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on clinic supplies with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with clinic supplies. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, afternoons. • Fully remote, with an occasional day on site in Nashville. • We will tell you what to expect at each step of the hiring process.
