Registered Nurse - care coordination (remote)

Fernwood Forge Transit Planning plans bus and rail service for cities around Omaha. We are hiring a Registered Nurse to take on care coordination. Work is collaborative and map-heavy. What you will do • Support work on survey data with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with survey data. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Omaha. • Applications are reviewed on a rolling basis.
