Registered Nurse - care coordination (remote)

Fernwood Mill Care Services supports families and individuals in Jacksonville with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on licensing with clinical expertise. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Jacksonville. • We reply to every applicant.
