Registered Nurse - care coordination (remote)

Summit Fern Care Services is a social-care provider serving older adults and people with disabilities around Memphis. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on care plans with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, mornings. • Remote-friendly, with a monthly team day in Memphis. • Applications are reviewed on a rolling basis.
